Add CLAUDE.md with role, target version (R1.03) and lookup order, and
top-level folders for skills, shared knowledge, examples and tools.
Move the document map to knowledge/ and rewrite the README.

Markdown version of the English page of ciserver.pdf, with IM numbers,
manual editions, and a note that the PDF links point to the default
local install path and are not stored in this repo.
